Welcome to Hawthorne Elementary.  We are excited to welcome you to our husky pack.  

To enroll you child into Kindergarten through 5th grade for the 25-26 school year at Hawthorne Elementary, please complete the enrollment steps below.

Step 1: Confirm we are your neighborhood school!  Who is eligible to attend Hawthorne Elementary?

• Students who live in our Hawthorne Enrollment Area - please check your home address HERE to see if Hawthorne is your neighborhood school.

• Students who have applied and been accepted through the Choice program

• Students new to the district, space permitting

 Step 2: To register your child you will need:

• Birth certificate

• Vaccination record with all immunizations current (DTap x5, Polio x4, Hep B x3, MMR x2, Varicella x2)

• Proof of residence such as a lease agreement, mortgage statement, SDGE bill, water bill or cable bill with your name and address listed

• School Verification (most recent report card)

• IEP *if applicable

• Universal form

• Enrollment form*editable form on your computer

• Home Language Survey*editable form on your computer

• Health Information Exchange

• Housing Questionnaire 

• Proof of a dental visit after age 4 Dental Form **Only required for our TK and Kindergarten students**

Families are encouraged to enroll using our online platform starting March 15th using by clicking here.  You are always welcome to complete the online forms and email them to the school at [email protected] or come in to complete the packet in person.  Enrollment hours are 9:00-11:00AM Monday-Friday.  We are always happy to help.
